Partners in Care: Better care starts with a seat at the table. Partners in Care helps primary care practices bring patients and families into the room where decisions are made. Together, they pick goals, try small changes and share what works.

At a glance (sample data): 40 practices in cohort one; 6 goals to choose from; 12 months, one full cycle.

The six goals:

  1. Patient and family voices. Patients and families help shape how the practice runs.
  2. Shared decision-making. Clinicians and patients choose treatments together.
  3. Patient activation. Patients have the knowledge and confidence to manage their health.
  4. Patient portal use. Patients can see their records and reach the care team online.
  5. Health literacy. Every handout, form and conversation is easy to understand.
  6. Medication management. Patients and families get help to take medicines safely.

How to get started:

  1. Join a kickoff call. Meet the team and see how it works.
  2. Pick two goals. Choose the two that matter most to your patients.
  3. Track progress. Share results each quarter and plan what is next.

What patient and family engagement looks like in practice.

The empowered patient says:

  • I feel seen, heard and cared for.
  • I’m asking the right questions.
  • My doctor has my accurate medical information.
  • I have ideas for improvements.
  • It feels good to help improve health care.

The empowered clinician says:

  • Great questions!
  • I’m listening.
  • I have the information I need.
  • We’ve avoided unnecessary procedures and costs.
  • My patients’ input has helped me improve my practice.
  • It feels good to work as a team with my patients.

Two voices, one team: care is better when patients, families and clinicians work together.

Let’s tell your program’s story together.
